The following list describes the enhancements and modifications implemented from 2012 through 2019, with the most recent changes listed first. More recent changes are described here.



Build 5.19r (29 December 2019)
Printed Labels: Added the ability to print a sheet of customized labels in Avery xx60 format (30 labels per page). The user has the option to print a full sheet of 30 labels for one worker, or 15 labels for each of 2 workers, or 10 labels for each of 3 workers. The content of each label is determined by the user.
TB Questionnaires: Added a button enabling the user to view the most recent Questionaire completed by an Employee via the Employee Portal.
Infectious Disease Surveillance: Added Quantiferon Gold Plus to the roster of selectable titers. Qiagen has notified its customers that all QFT Gold testing will now be processed using the Gold Plus protocol. This results in a report of four individual titers (NULL, MITOGEN, TB1, and TB2). Interpretation of the test results as "positive" or "negative" depends on evaluation of the four values.
Drug Testing: Made additional revisions to the screen layouts for documenting specimen collection/screening (and reporting to the Employer or DER), laboratory results, and MRO verification (including reporting to the Employer or DER).
Drug Testing Reports: Updated the "boilerplate" wording of printed reports generated for a DER or Employer.
Clinical Notes: Modified the sequence of screens enabling a clinician who is not the author of a note to review the note and make additional comments.
Diphtheria/Pertussis Status Roster: Revised the wording of reports to reflect current CDC recommendations: if an Employee was last vaccinated more than 10 years ago, the roster will report the date of the last immunization with the notation NO DOCUMENTED IMMUNITY.
Facility Holidays: Updated the sequence of documenting the dates on which the facility is closed.


Build 5.19q (8 December 2019)
Drug Testing: Revised the screen layouts for documenting specimen collection/screening (and reporting to the Employer or DER), laboratory results, and MRO verification (including reporting to the Employer or DER).
Customization Requests: Modified the software code to implement two new facility-specific report forms requested by an individual Employee Health director. The modifications do not affect other MediTrax users.


Build 5.19p (19 November 2019)
Follow-up Appointment Scheduling: Modified the program code to eliminate occasional scheduling of duplicate appointments on the same day.
Respirator Fit Testing: Corrected the format of "Who's Due" reports to ensure that individuals whose medical clearance or fit test has no expiration date are not included in the report.
Vital Signs: Enabled entry of numeric values in all fields, even when the user has placed the cursor in the middle of the field rather than at the beginning.
Error Correction: Investigated and resolved a Build 5.19m error message reported by a MediTrax user.

Authorized MediTrax Users: Revised the display that appears when Show Former Users is clicked.
NHSN Immunization/Declination Summary: Corrected the header line of the printed report.
Seasonal Flu Questionnaire/Consent Form: Added the Employee's home or cell phone numbers to forms in cases where no work phone was documented.
First Report of Injury: Updated the Physician's and Employer's First Reports of Injury for several states.

CDC Vaccine Information Statements (VISs): Added newly-released VISs for Chickenpox (Varicella), Hepatitis B, Intranasal Flu, Japanese Encephalitis, Meningococcal ACWY, and Meningococcus Serogroup B (MenB) vaccines to the MediTrax library.
Flu Vaccine Questionnaire/Consent Form: Updated the form for the 2019-2020 immunization season.
Data Backup Options: Added options to create backup files of all documents in the SCANS subfolder, as well as all Clinical Notes in the database.
FMLA Case Documentation: Corrected a small "glitch" in the option to attach a scanned document to a specific FMLA case.
Error Correction: Investigated and resolved a Build 5.19f error message reported by a MediTrax user.


Build 5.19g (9 August 2019)
FMLA Current Cases Roster: Added the option to limit the roster to approved cases or pending cases.
FMLA Documentation: Added a Scans button to the data entry screen, enabling users to attach electronic documents to an Employee's FMLA record.
TB Questionnaire/Survey: Added the option to change the questions displayed on the Questionnaire. Individual Employee responses are now tracked according to the questions which were displayed on the data the Questionnaire was completed.
Main Menu Screen: Added our new MediTrax logo to the screen.


Build 5.19f (29 July 2019)
Clinical Notes Template: Streamlined the process for creating/updating user-created text templates.
Employee Portal: Updated the code for installation and operation of the Employee Portal. NOTE: The Portal application is currently being revised and updated to enable Employees to log in remotely (rather than only logging in from a network location).
Orchard LIS Interface: Completed development of a pilot interface with the Orchard® Harvest™ Laboratory Information System, enabling the user to order titers and other lab tests directly from within MediTrax, and to retrieve reported results from HL7 files created by the LIS.
Financial Account Review: Modified the process for printing a summary of posted charges (only available to facilities which invoice for their services).
Service Charge Posting: Resolved an error which in rare circumstances limited the use of multiple fee schedules (only available to facilities which invoice for their services).


Build 5.19e (17 July 2019)
Respirator Fit Testing: Modified the "Who's Due" Roster to ensure that the presence of an outdated fit test report does not incorrectly result in the Employee being included on the Roster if there is a later, current fit test.
Fit Testing "Who's Due" Roster: Added the ability to select a timespan for the Roster. If an Employee was fit tested during the specified timespan, he/she will not be included on the Roster.
Fit Testing "Who's Due" Roster: Modified the format of the roster to eliminate irrelevant columns, and changed the wording of the Employee's status to either DUE FOR EVAL or NO FIT TEST RECORDS ON FILE.
Case Manageement / Occupational Injury: Resolved an error which prevented changing the category of an incident from Injury to Exposure and vice versa.
Linking a Scanned Document: Modified the code which directs Adobe Reader (or another PDF reader) to display the content of a PDF file when the Click here to view the .PDF document square is clicked.

Error Correction: Investigated and resolved a 5.19c error message reported by a MediTrax user.

Treatment of Latent TB Infection (LTBI): Added the option to document the treatment of LTBI in an employee's EMR.
TB Symptom Review: Added the ability to document the date of TB symptom review in an employee's EMR.
TB Questionnaire: Added the ability to modify the questions displayed in the Questionnaire. The version of questions is tracked for each Questionnaire completed.
MMR and Varicella Surveillance Clinical Rosters: Adjusted the printed rosters to ensure vaccines and titers are displayed in chronologic order.

IAC Immunization Guide: Added a link to the IAC Guide to Contraindications & Precautions to Commonly Used Vaccines, 2019.

TB Screening Recommendations: Added the CDC Recommendations for TB screening, testing and treatment of health care personnel (5/17/2019) to the MediTrax forms library.
Inservice Training: Modified the process for viewing an Employee's training record, enabling the user to click on an individual training course to display the details of that specific course.
Clinical Encounter Notes: Resolved a user-reported error which in some cases prevented creating a new Encounter note.

Vaccine Administration Guidelines: Added this IAC document to the library of stored forms.
Respirator Fit Testing: Placed the documentation process in a loop to enable creating records for multiple workers without having to return to the main program menu.

Onboarding Status: Corrected a flaw in the search parameters to ensure that TST dates are correctly displayed, and that seasonal flu immunizations administered after the beginning of the current flu vaccination season are tallied.
OSHA Respirator User Questionnaire: Added a link to enable review of an Employee's questionnaire.
Hepatitis C PCR: Added a synonym HCV Quantitative PCR [NAAT] to the picklist of infectious-disease surveillance events. The PCR assay Hepatitis C RNA - PCR Quantitative was previously in the picklist, but some users were unclear that the two terms refer to the same test.

Onboarding Status: Added the option to view common onboarding requirements for newly-hired Employees (drug testing, respirator clearance and fit testing, immunizations and titers) in a single interactive window accessible via the Current Status tab in the EMR. The option is available for all Employees hired in the past 60 days. Requirements which have been met are displayed in green; those still outstanding are displayed in red.
   
NOTE: The example shown above uses the onboarding requirements at four hospitals which provided suggestions for the design of the display. If your requirements differ, please contact support@meditrax.com to see if the display can be customized for your facility.
NHSN Reporting: Added the ability to flag one Employee, or selected individuals from a subgroup of Employees (an entire Department, or an entire Cost Center) as NHSN EXEMPT.
Recall and Appointment Notification via Text Messaging: Added the ability to document an Employee's cellular service provider, in order to facilitate notification of upcoming recall reminders and appointments, as well as missed appointments and overdue recalls, via text message.
BBP Exposure Guidelines: Added a link to this document within the data-entry screens for documentation of a BBP Exposure.
Seasonal Influenza Vaccine: Added Afluria quadrivalent IIV4 to the picklist of infectious-disease surveillance items.
HIV4G Testing: Added an item HIV-1/2 Antibodies + p24 Antigen to the picklist of infectious-disease surveillance events.
Hepatitis C Immunoassay: Added an item HCV Immunoassay (EIA) with Reflex to Quantitative PCR to the picklist of infectious-disease surveillance events.
Respirator Clearance / Fit Test Rosters: Resolved a user-reported error which in specific circumstances omitted individual employees from rosters.

Demographic Record Update: Resolved a "bug" which occasionally caused the update process to fail to add a new Employee.
Service Quantity: Resolved a "bug" which prevented changing the quantity of a specific service when documenting an Encounter.

Community TB Risk Toolsheet: Added the CDC Risk Toolsheet to the library of MediTrax forms.
Audiometric Testing History: Revised the printout to ensure that STS is always displayed for any audiogram with evidence of a Standard Threshold Shift.

"Sick Call": Expanded the parameters for documentation of "sick call" events.

Documenting Flu Vaccine Declination: Modified the alignment of lines in the display for better clarity.
Encounter Documentation: Implemented the ability to modify the quantity of a previously-documented (but uninvoiced) service.
Payor/Insurer Demographics: Added the ability to specify, for each payor, whether the SSN should be hidden, masked (last 4 characters only), or shown in its entirety.
MediTrax-format Invoices to Payors/Insurers: Added the name of the Employer to each encounter.
Error Correction: Hepatitis A immunization: Modified the algorithm for evaluating immune status for each individual.
Error Correction: Account Review: Corrected an error which resulted in failure to display uninvoiced services.
CMS 1500 Invoices: Modified the display of boxes 32 and 33 to show longer names in a reduced-size font.
CMS 1500 Invoices: Modified the display of box 31 to allow room for the Clinician's signature.
Invoices: Added the ability to indicate multiple quantities of a selected service (e.g., "2 @ $30.00"), with the total charge shown as $60.00.

Employee Portal: Resolved two user-reported error messages which interfered with setup and functionality of the Employee Portal.
Group Immunization ("Flu Clinic"): Deleted the automatic addition of a record in each Employee's immunization history showing the date on which the VIS was provided.
Seasonal Flu Immunizations: Added the season covered by a Clinical Status Roster to the header of the printed report (e.g., 2018-2019).
Seasonal Flu NHSN Report: Revised the reporting algorithm to ensure all newly-released vaccine options are correctly tallied in the Report.
DOT Medical Exam Forms: Updated the application to include the recently-revised MCSA-5875 and MCSA-5876 forms.
DOT Medical Certificate: Added the option to print the updated Form MCSA-5876 at the time of "One-Minute Checkout".
Inservice Training: Resolved a user-reported issue with storage and retrieval of training course rosters.
Invoices for Employer-Requested Services: Added the option to hide Employee SSNs on invoices to Employers and Insurers.
Physician's First Report of Injury (California): Made minor formatting changes to the CA-5021 printout.


Build 5.18h (24 September 2018)
Medical Work Restrictions: Updated the display of the work restrictions link in the Safety | Injury / Illness / Exposure Documentation | Employee Work Status tab.
Hepatitis A Surveillance: Updated the criteria for determining policy compliance to correctly reflect the status of an employee who has received three doses of TwinRix, or two doses of Twinrix and one dose of Hepatitis A vaccine.
Tuberculosis Screening Questionnaire: Revised the "default" questions to be included on the questionnaire, with the option for an individual facility to modify the sequence and wording of questions.
Tuberculosis Screening Questionnaire: Added the ability to print a blank paper questionnaire, or to print a completed questionnaire.
Employees Transferring From One Facility to Another (separate MediTrax databases): Added the ability to create an HL-7 file which may be sent to the new facility in order to import all clinical records associated with that Employee.


Build 5.18g (24 August 2018)
Department Roster: Implemented the option to edit the roster of Departments at a specified Employer by selecting Utilities | Master Data Tables | Departments.
Vaccine Information Statements: Added the newly-released VIS for Meningococcal ACWY (8/24/18) to the on-screen library.
Roster of Job Titles: Implemented the option to edit the roster of Job Titles at a specified Employer by selecting Utilities | Master Data Tables | Departments.


Build 5.18f (5 August 2018)
Initial/Periodic Evaluations: Implemented the ability to define an unlimited number of initial and periodic evaluations, with the option to specify the frequency each evaluation should be repeated.
Initial/Periodic Evaluations: Added the option to generate a "Who's Due" roster of Employees who are due for any specified type of initial or periodic evaluation within the next 30 days.
Immunization Policy Compliance: We "tweaked" the reporting mechanisms for the policy-compliance rosters to ensure that when a user wishes a roster which includes only Employees who have not complied with the facility's immunization policies for Hepatitis A, Hepatitis B, MMR, Pertussis/Tdap, Tetanus/TD or Varicella, that the roster includes only those individuals -- and only if they are current Employees.
Immunization Policy Compliance: We received requests from several users asking that the processes MediTrax uses to define "policy compliance" are clearly explained (since "policy compliance" is clearly not the same as "immunity" or "presumed immunity"). In each case, the criteria used to determine "policy compliance" are displayed on a pop-up screen at the beginning of the process.


Build 5.18e (29 July 2018)
Tetanus Policy Compliance Roster: Added this report, enabling identification of Employees who have not received a tetanus (or TD) booster, or Tdap vaccine, within the past ten years.
Infectious-Disease Policy Compliance Rosters: Made minor revisions to reports enabling identification of Employees who have not been adequately vaccinated (or demonstrated immune titers) for Hepatitis A, Hepatitis B, MMR, Tdap and Varicella. When the reports were first released in Build 5.18d, the filter to include "only non-compliant employees" was not reliably implemented and some rosters included all employees.
Job Requirements: Implemented a comprehensive picklist of job tasks and physical requirements, enabling documentation of the frequency with which each task or requirement is necessary. This function is available under the Safety tab.
Appointment/Recall Reminders: Modified the display of appointments and reminders for an individual Employee, simplifying the process of emailing the Employee and his/her Supervisor.
Appointment/Recall Reminders: Added a display line indicating whether the Employee's email address and Supervisor have been documented, and added a button enabling the user to review and edit the demographic record (e.g., to document the email address and/or Supervisor) and return immediately to the reminder picklist.
Appointment/Recall Reminders: Added two mouse-clicks enabling appointments and recall reminders for a single Employee to be filtered so the display (optionally) includes only a selected category of appointment/reminder, and (optionally) only appointments/reminders scheduled within a defined time frame.
Combining Demographic Records: Modified the process for combining demographic records when a SSN belonging to a previously-entered record is entered for a new Employee. If the records belong to the same individual, the process now enables the User to update the previous demographic record with the newly-entered information, or to retrieve the information from the previous record. In either case, the clinical data (immunizations, fit tests, etc.) from both employment records are combined into a single record for the individual. If the Employee has two or more employment records, the Employer-specific data fields (job title, department, Payroll ID, hire/term dates, etc.) remain unchanged for each record.
MediTrax "Overview" PowerPoint: Uploaded to our main web page the PowerPoint presentation which we routinely send to prospective software clients. Several facilities had requested that this be available as a brief overview of MediTrax for new staff members.


Build 5.18d (9 July 2018)
"Who's-Due" Rosters: Added the ability to automatically create individual Recall Reminders when generating a "Who's-Due" roster of Employees who are either due or overdue for TB surveillance, or who are non-compliant ("status unknown") with seasonal flu immunization policy.
Infectious-Disease Policy Compliance Rosters: Added reports enabling identification of Employees who have not been adequately vaccinated (or demonstrated immune titers) for Hepatitis A, Hepatitis B, MMR, Tdap and Varicella. We realize that different facilities may have different policies regarding immunization and testing (and it would be counterproductive to keep notifying a Supervisor that an Employee who ALWAYS declines a vaccine that the Employee is "due for evaluation") — so the criteria used in preparing each report are displayed when the roster is requested.
"Employee Encounters" tab in EMR dashboard: Decreased the height of the Encounter picklist to allow the Add button to remain visible (it was hidden after the addition of two additional tabs in Build 5.18b). .
VIS Documentation in Printed Immunization History: Added the option to omit VIS documentation from the printed summary. Many facilities document that a VIS was provided with each immunization
Policy Compliance Rosters (Hepatitis A and Varicella): Modified the selection algorithm to enable inclusion of only non-compliant Employees in the roster when requested.


Build 5.18c (1 July 2018)
Data Access Audit: Implemented the ability to export the data-access audit roster to Excel.
Immunization Verification Certificate: Added the name of the vaccine manufacturer to the Certificate.
Documenting Immunizations/Titers/TB Tests: Added a "delay" in activating the Accept button to ensure at least one event is listed in the right-hand list of selected events, to eliminate an error message which was displayed when the Accept button was clicked before any selected items were displayed in the right-hand list (usually occurring with slow server response times).
Facility Revenue Summary: Resolved an error in the report process (in facilities which bill for services).
Employee Portal Form Completion: Modified the Portal-update process to ensure that all forms completed by an Employee (Flu Questionnaire/Consent, OSHA Respirator User Questionnaire, and TB Survey/Questionnaire) are synchronized with records stored in MediTrax.


Build 5.18b (15 June 2018)
Data Access Audit: Revised the description of data-access events. Event categories now include:

Build 5.18a (3 June 2018)
"Not Responding" Messages: Found and deleted a single line of code (!) which was found to be slowing the display of progress bars. Our tests indicate this will speed the display of progress bars significantly and should eliminate about 90% of "Not Responding" messages.
Immunization Policy Compliance Rosters: Revised the algorithm for determining which Employees are "policy compliant", and added a pop-up message explaining the algorithm:
  - An Employee who has EVER received the vaccine (or has completed the vaccine series) will be listed as "policy compliant".
  - An Employee who has EVER declined the vaccine will be listed as "policy compliant".
  - An Employee whose record contains documentation of titer results will be listed as "policy compliant".
Note: this revised algorithm does NOT apply to policy compliance determinations for seasonal influenza and tuberculosis. Seasonal influenza policy compliance will continue to be determined each year, and tuberculosis screening/testing compliance will continue to be determined for the time period selected by the user.
IT Access to MediTrax: Added the ability to provide IT/IS staff members with limited access to the application in order to run Demographic updates from an HR extract. As before, access as an Authorized User is not required to perform software updates.
Authorized Users: Added the ability to document the professional credentials (MD, DO, RN, NP, PA, etc.) of Authorized Users.
Incomplete TB Testing: Changed the wording of the status of Employees who have not completed required initial TB testing from NEEDS 2-STEP TESTING to REQUIRES TESTING.
Infectious-Disease Surveillance: Added Hepatitis C RNA - PCR Quantitative to the list of titers which may be documented.
Infectious-Disease Surveillance: Revised the algorithm for reporting Varicella immunity. A positive titer (documented as either Varicella Titer or Varicella Titer - IgG) will be regarded as evidence of immunity.


Build 5.17z (5 May 2018)
Employee Demographics: Added the ability to document a secondary Department, Cost Center and Job Title (in addition to the "home" Department and Cost Center, and primary Job Title) for an individual Employee.
Infectious Disease Surveillance: Added Heplisav-B (Recombinant) to the picklist of vaccinations which may be documented.
Infectious Disease Surveillance: Added fields for Location and Cost Center to the Excel spreadsheet report of all infectious-disease surveillance events for a specified period.
Duplicate Employee Demographic Records: Modified the code for combining duplicate demographic records, to improve accuracy of the process in all cases.
NHSN Reporting for Seasonal Influenza: Added the ability to view or print the names of Employees whose status is "unknown" but who do not appear in the "Who's-Due" Roster because their employment has terminated.
Displaying Appointments and Recall Reminders: Resolved an error message which appeared when a specific unusual combination of parameters was entered for the on-screen display.

Infectious Disease Surveillance: Added CT Scan (chest) to the picklist of surveillance events.
Hepatitis B Clinical Status Report: Sorted the dates of Hepatitis B immunizations and titers in chronological order.
MMR Clinical Status Report: Sorted the dates of MMR immunizations and titers in chronological order.
CPT Modifiers: For occupational health clinics which submit invoices to carriers and clients, added the ability to modify the CPT-4 code and modifier(s) for each selected service provided.

Merging Duplicate Demographic Records: Revised the algorithm for identifying duplicate records to enable merging of records with either (a) the same SSN or (b) the same Payroll ID number.
Configuration File: Enabled the use of a MT5.INI configuration file to ensure the application utilizes the correct data folder(s) when the desktop shortcut points to a folder identified by its UNC location (e.g., \\myserver\meditrax\live).
The format of the MT5.INI configuration file is:

Tuberculosis Surveillance: Changed the on-screen display of TSTs. The date of application is displayed EXCEPT in cases where the date of application was not documented. In those cases, the date the test was read will be displayed in the format 8/18/17 (R). As before, both dates will be shown on the printed immunization-record summary.
Immunization Record Summary: Added a pop-up reminder that the printed summary may not contain all records in the database; specifically, seasonal flu immunizations will only be shown for the past 3 years; HIV and Hepatitis C titers will not appear on the printed summary; and negative TB skin test results will only be shown for the previous 3 years (except that the last TST result will always be shown even if it was more than 3 years ago).
Duplicate Incident Records: Added a menu option to merge the records of two Incidents, in cases where two or more records have inadvertently been created for the same Incident. This process preserves all Encounter records, Clinical Notes, Case Management events, and documented services for both Incident records and merges them into the single remaining record.

Tuberculosis Surveillance: Clinical Status Roster and "Who's-Due" Roster: Implemented a comprehensive revision of these reports to better account for multiple tests. The program logic is now this:
MediTrax first asks you to confirm the "leeway" period during which testing performed prior to the date of hire will be considered in determining whether the Employee is due for (re)evaluation. The default "leeway" is 30 days.
If the Employee was hired/rehired more than 13 months ago:
 - If the Employee's last TST or IGRA test or clinical followup was more than 12 months ago, the Employee is now OVERDUE FOR EVAL.
 - If the Employee's last TST or IGRA test or clinical followup or questionnaire was more than within the past year but more than 11 months ago, the Employee is DUE FOR EVAL PRIOR TO (one year after the last evaluation).
 - If the Employee's last TST or IGRA test or clinical followup or questionnaire was within the past 11 months, the Employee is NOT due for evaluation.
If the Employee was hired/rehired within the past 13 months:

 - If the Employee had TWO TSTs (or one IGRA and then one TST) within the past 12 months plus the "leeway" period, and the most recent TST was less than a year after the previous TST, and the most recent TST was less than a year after the date of hire, and the most recent TST was more than 11 months ago, the Employee is DUE FOR EVAL PRIOR TO (one year after the last evaluation). The Employee's hire date is displayed on the "Who's-Due" Roster.
Tuberculosis Surveillance: Changed the on-screen display of TSTs to show the date the test was applied rather than the date it was read. This is consistent with CDC recommendations and with the display of other events in an Employee's infectious-disease surveillance history: titers are displayed with the dates they were drawn, rather than the dates on which the results were reported by the lab; and TB questionnaires and surveys are displayed with the dates they were completed, rather than the dates on which they were reviewed.
Surveillance Group Rosters: Added the ability to double-click on any Employee name in the Roster in order to access the Employee's demographic data-entry screen. Upon exiting the demographic screen, MediTrax returns to the Surveillance Group roster display.
Surveillance Group Rosters: Added the ability to open the demographic record or the EMR of any Employee listed in a Surveillance Group roster by highlighting the Employee's name and clicking the D or E buttons in the bottom left corner of the screen. Upon exiting the demographic or EMR screen, MediTrax returns to the Surveillance Group roster display.
Pop-Up Appointment/Recall Reminders: Added the display of any user-created free-text note when displaying the notice of any missed appointment or upcoming appointment/recall within the next 30 days. This notice pops up whenever opening the clinical record of any Employee with such an appointment or recall reminder in the database.

NHSN requires reporting for all Health Care Providers who were physically present in the facility between October 1 and March 31. If you wish to exclude an Employee from your NHSN statistics and "Who's-Due" reports, you may document FLU VACCINE - NHSN EXEMPT (with the option to enter a specific reason) in the Immunization Record of that Employee.
MediTrax tallies all vaccinations and declinations occurring between August 1 and June 30 for the group of Employees working during the October-to-March time frame.
Department Listing: Added the ability to delete a record from the Department picklist. If any Employees are assigned to that Department, MediTrax offers the option to reassign those Employees to another Department.

MediTrax now compiles a list of all individuals physically present in the facility between October 1 and March 31 (as determined by payroll status or hire/termination dates, and excluding all individuals with NHSN EXEMPT documented in their immunization history during that time period), and then seeks all influenza vaccination/declination events occurring between August 1 and June 30 for those individuals.
Flu Immunization Compliance Summary: Modified the parameters of Compliance Summaries. To prepare a Compliance Summary, you now define the time period during which compliance was required. The Summary will include only CURRENT Employees. It will not include individuals whose employment began after the end of the compliance period, nor will it include terminated Employees. This Summary will tally ONLY vaccinations and declinations which occurred during the selected timespan.
Flu Immunization "Who's-Due" Roster: To generate a report of CURRENT Employees who have not yet received or declined this year's seasonal influenza vaccine, you may generate a "Who's-Due" Roster which tallies all vaccination/declination events occurring between August 1 and June 30 and eliminates those individuals (as well as all individuals with NHSN EXEMPT documented in their immunization history) from the DUE FOR EVAL category.
Bloodborne Pathogen Exposures: Eliminated the tabs for Employee and Source Screening from the "Sharps" Incident data entry screen. Those tabs are available on the Bloodborne Pathogen Exposure data entry screen.
Bloodborne Pathogen Exposure: Modified the screen display of the immunization/surveillance history for both the Employee and the exposure Source. After clicking Update, the left side of the display screen shows Tetanus/TDAP and Bloodborne Pathogen surveillance events as before, while the right side of the screen will enable documentation of EPINet and OHSN data (this is currently under development).

For security reasons, Windows® 7 and later versions will not display a Help file which is stored on a network drive.
TST Reporting: Resolved a software "bug" which at times required double-entry of the date on which the test was read when the "read date" was 3 days after the application date.

Pressing <Ctrl-F5> from the Main Drop-Down Menu screen reduces the size of the display font, while pressing <Ctrl-F6> increases the font size.
OSHA 301: Added a button to the OSHA Reporting tab of the Incident Documentation/Investigation data entry screen for U.S. customers, enabling the user to print an OSHA 301 form directly from that screen.

Incident Documentation & Tracking: Activated the BBP Exposure and "Sharps" Incident checkboxes on the Incident Documentation screen. Selecting either or both of these checkboxes activates the corresponding button at the bottom of the window, enabling seamless transitions between screens to document these types of injuries and exposures without having to return to the Main Menu screen.

Flu Clinic Documentation: Added a drop-down menu box to enable selection of the type of vaccine administered when multiple vaccine options (e.g., Flu Vaccine - Injection and Flu Vaccine - Intranasal have been selected during the process of configuring the Flu Clinic information.
Flu Immunization - Reporting Exemption: Added a Surveillance Event Flu Vaccine - NHSN Exempt to enable Employees who were on leave (sick leave, maternity leave, long- or short-term disability leave, military leave, etc.) to be excluded from the numerator and denominator of NHSN reporting.
Flu Immunization Compliance Reporting: Revised the filtering code to eliminate all terminated Employees from Compliance and "Who's-Due" Rosters even if they were on the payroll during the October 1 - March 31 immunization period.
Flu Immunization - NHSN Reporting: Revised the default beginning and ending dates of the immunization/declination reporting period to October 1 and March 31 (regardless of the actual timespan during which the facility provided the vaccine to Employees), in compliance with updated NHSN criteria.

Daily Office Schedule "NOTES" Column: Added the ability to create a column in the Daily Office Schedule containing free-text notes for any time block. The column may be created by selecting Demographics | Staff Clinicians and adding a record with the last name NOTES.
The NOTES column may be moved to the right side of the Daily Office Schedule display, or may be hidden entirely, by selecting Utilities | Schedule Customization | Clinician Display Sequence.
Clicking on any blank cell in the NOTES column enables the user to create a free-text note which will be displayed in that cell. Clicking on an existing note enables the user to edit or delete the note.
Group Immunization - Flu: Modified the program code to automatically populate the vaccination date for each Employee with the date of the Flu Clinic.
Flu Immunization - NHSN Summary: Realigned the display of percentages in one column.
Clinical Protocols: Added the ability to assign a specific Payor (Guarantor) to each service in a Protocol.

TST Reading Date: Eliminated the automatic population of the date of TST reading with the current date.

Demographic Updates: Implemented a process to print a log of all changes to demographic records made in previous import/update routines during a specified timespan. The log is available by selecting Demographics | Employees | Demographic Data Import | Print Change Log.
SSN Display: Changed the default picklist parameter to show SSNs, without requiring selection of the SSN variable in the Utilities | Database Administration | Program Parameters | Demographics data entry window.
Exporting a Department Roster to Excel: Corrected a program error which prevented exporting the roster to a spreadsheet.
Financial Transaction Detail Report: Corrected a program error which in some instances resulted in a message that no service charges were posted on the specified date.


Build 5.14p (7 September 2015)
ICD10 Implementation: Completed the process of activating ICD10 selection. Users are now able to switch from ICD9 to ICD10 coding at any time. From the main drop-down menu, select Utilities | Master Data Tables | Diagnoses/ICD Codes and choose ICD10. You will then be able to select ICD10 diagnoses whenever you document the evaluation of an occupational injury or illness.
Medical Diagnoses: Modified the display of diagnoses to fit the longer ICD10 text descriptions into the text box. Diagnoses are now prefaced with the ICD code.
Medical Diagnoses: Modified the behavior of the text box containing a medical diagnosis to display a pop-up message containing the entire ICD code and diagnosis.
CMS-1500 Invoices: Updated the printed CMS-1500 format to the most recent version authorized for use with both ICD9 and ICD10 coding.
Comprehensive Immunization Summary: Implemented the ability to export a comprehensive summary of vaccinations, declinations and titers to a spreadsheet. The summary may be configured to include any user-specified demographic group and any defined time span.
Employee Portal Configuration: The Portal configuration-update process may be automated to run overnight by creating a command on the Windows Server Task Scheduler using the format

This simplifies the process of updating the demographic roster stored on the Portal, since the update must be performed when no one is actively logging in to the Portal.

Build 5.14n (18 August 2015)
Infectious-Disease Panels: Implemented the ability to store various combinations of immuizations and testing events in a variety of user-defined panels (e.g., Preplacement Evaluation or Post-BBP Exposure. Subsequently, the panel events may be selected as a group without the necessity to select each event individually when adding immunizations and/or tests to an Employee's record.
Documentation of Titer Results: Added the ability to document whether numeric values represent IU/mL (International Units per milliliter) or "relative value" results (in which case the laboratory value represents the amount of antibody/antigen relative to a "cutoff" level which the lab has determined to represent a "positive" result. Values of 0.9 or less usually represent "negative" titers, with 0.91 to 1.0 in the "equivocal" range and any result above 1.0 representing a "positive" titer. A pop-up Help button displays a reminder of these two types of results.
Software Customization: Implemented numerous facility-specific customizations and enhancements requested by indvidual MediTrax users.


Build 5.14m (8 August 2015)
CDC Vaccine Information Statements: Updated the MediTrax VIS library with the documents released by CDC on August 7 2015.
Employee Demographic Data Entry: Enabled the use of SSN/SIN numbers beginning with 00000 or 11111 in facilities which do not wish to store complete account numbers in the Employee data table. Note that when such numbers are used, MediTrax will not perform its usual check to determine if the SSN/SIN number has already been assigned to another Employee.
EMR Current Status tab: Enabled users to define their own text description of the dated events displayed at the top of the Current Status tab.
Flu Consent Form: Enabled an electronically-signed Flu Consent form to be "locked" (e.g., at the time of a Paperless Flu Clinic) so it cannot be changed subsequently.
Outstanding Invoice Balances: Revised the algorithm for recalculating the outstanding balance on an invoice after credits and adjustments are posted.
Temporary Data Tables: Changed the location for creation of temporary data files (beginning with the character $) to the folder MT5_TEMP on the C: drive of the user's workstation. This change is intended to minimize the creation of dozens or hundreds of temporary files on the network share, which in some cases was shown to increase drive space fragmentation and slow program performance.
ICD10 Readiness: Made minor modifications to the drop-down selection screens for users to test and submit recommendations for greater ease of use.


Build 5.14k (25 July 2015)
Drop-Down Menu Display: Modified the display of the MediTrax drop-down menus to utilize a larger font size for better visibility. We have been working for many months to develop a "workaround" for the Windows® requirement to display screen headers and menus in the small system-default font.
ICD-10-CM Implementation: Added a menu option enabling users to test a routine for selecting an ICD-10-CM diagnosis utilizing sequential "drill-down" picklists containing the 2015 official code revisions. This routine is now available as a beta-test option in the Utilities menu. Your comments and suggestions are welcome as we prepare to make ICD-10 functionality available to all facilities wishing to convert their recordkeeping to this new coding system, currently scheduled for required implementation by October 1 2015 in all CMS invoicing systems.
NOTE: Many of our customers who do not invoice for their services have indicated their preference to continue using the current (and much simpler) ICD9-CM coding system to document diagnoses. This will continue to remain an option in MediTrax.
OSHA 300 Log: Added the ability to prepare an OSHA 300 Log, as well as an OSHA 300A summary, limited in scope to recordable injuries and illnesses occurring at a specified Reporting Establishment (i.e., a business entity which is distinct from the parent Employer).
Employee Immunization Record: Modified the algorithm for printing TB skin test results so the printed summary now includes all tests in the past three years as well as the most recent test (regardless of date) if it has been more than three years since the last test.
Employee Immunization Record: Modified the algorithm for printing seasonal influenza vaccinations so the printed summary now includes only "flu shots" in the past three years.
Post-BBP Exposure Test Roster: Added a Clinical Status roster option listing all Hepatitis C and HIV titers for a selected demographic group. The optimum utilization of this report is to track follow-up testing for all individuals in a BBP Exposure or "Sharps Injury" Medical Surveillance Group.
Random Selection for Drug Testing: Added the ability to specify the date on which the next random draw will take place, enabling the calculation of recommendations for the number of individuals to be selected in the current draw in order to maintain required testing percentages until the date of the next draw.
Flu Consent Forms for 2015-2016: Implemented the ability to specify the wording of questions to be asked on Flu Consent forms for the 2015-2016 vaccination season.
Respiratory Protection: Added a link to the "Diseases and Conditions" informational website prepared by the California Department of Health and Human Services.
Data Exports to Excel: Modified the message displayed after saving MediTrax data to an Excel file, reminding the User that newly-created spreadsheets are stored to the default location specified in the Excel configuration options (which is usually the User's MY DOCUMENTS folder).

Paperless Flu Clinics: Several modifications to this function have been made as a result of user comments and suggestions during our initial webinars on this subject. We modified the display presented to Employee Health staff when each Employee arrives to participate in a flu clinic: buttons have been added to enable display of the Employee's immunization record, to enable creation of a free-text note related to the current immunization or declination, to change the manufacturer and lot number of the vaccine(s) being used for the remainder of the flu clinic, and to document that the vaccine was received on a different date.
Paperless Flu Clinics: Modified the display presented to Employee Health staff when an Employee indicates he/she has received the vaccine elsewhere: instead of a button reading I CONSENT to receive the vaccine, the consent wording now reads I RECEIVED the 2013-2014 vaccine elsewhere.

You may, of course, "trigger" the import at any time, or activate it on any day for unattended completion between 10 p.m. and 4 a.m. that night. The purpose of creating a schedule is to generate a message which will be displayed on "scheduled" days to individuals with appropriate privileges, reminding them to activate the unattended import process before logging off.

"Undoing" Pre-registration for "No-Show" appointments: Enabled the "undoing" of a pre-registration if the Employee fails to keep the appointment.

Appointment / Reminder QuickPick Menu: Added a pop-up menu which is displayed when scheduling an appointment or creating a recall reminder for an Employer-requested evaluation. This simplifies scheduling a generic reevaluation (such as a periodic DOT exam or annual audiometry or TB testing). The Employer-Defined Evaluation option appears at the top of that menu so that just clicking "Enter" will move to the next screen containing your user-defined list of the specific evaluations you provide.

Email Reminder Notices: Implemented the (long-awaited) ability to send individual user-specified email messages to employees (at their work and/or home email addresses) as well as to their supervisors from a streamlined point-and-click menu, reminding each employee of future appointments as well as unscheduled recall reminders. The reason for the evaluation may optionally be added to each employee's reminder message, but the reason is never disclosed to the employee's supervisor. This function is available by selecting the

menu sequence and defining the scope and time frame of evaluations you wish to display.
This automated point-and-click email-reminder function has also been implemented for missed appointments. Select the

Recall Reminders: Added drop-down lists for the type of evaluation for which the employee should return. Added the ability to indicate whether the followup evaluation should take place on a specific future date, or simply during a future week, or during a future month. This function is available at the time of documenting any surveillance event (audiometry, spirometry, respirator fit testing, immunization, infectious-disease surveillance testing) or inservice-education program.
Occupational Injury Listings: Added the Workers Compensation Claim Number to various picklists (e.g., Encounter Documentation and Financial / Account Review).
Immunizations: Added a "Save these values" checkbox underneath the data-entry boxes for the vaccine manufacturer, lot number, expiration date, and vial-open date. Selecting this option will save those four values to automatically populate the respective fields the next time a vaccination is documented.
TB Surveillance Clinical Roster:
Modified the format of the Roster to display TST and IGRA positive tests, as well as History-of-Previous-Positive indicators, on a shaded background for faster recognition.


Build 5.08v (1 May 2012)
Infectious-Disease Surveillance: Added fields to the documentation for vaccinations and TSTs, enabling documentation of the date a vial of vaccine or Mantoux reagent was opened. Implemented drop-down picklists for the site of administration. Added a drop-down picklist for the source of vaccine (e.g., multi-dose vial or prefilled syringe). Added fields for the time each TST was administered and read, together with documentation of the clinicians administering and reading the test.
Free-Text Appointment Notes: Modified the content of each block of the Daily Office Schedule to display a ¶ symbol at the beginning of the second line when a free-text note has been created.
Enabled display of appointment/recall details (including free-text notes) by clicking an appointment or recall description in the Follow-Up Appointments/Recall Reminders list which is displayed at the time of documenting immunizations or medical-surveillance evaluations and tests.


Build 5.08u (19 April 2012)
"Paperless Flu Clinics": Further streamlined the documentation process, enabling on-screen completion of consent forms as well as point-and-click documentation of the reason for vaccine declination. When ID-badge scanning (barcode, magnetic-stripe or RFID/HID chips) is utilized for rapid paperless processing, an individual who has forgotten his/her ID badge may be selected from the Employee picklist. This function is available by selecting the

Screen Format: Continued the process of revising screen formats to display the screen "header" in large font (compensating for the smaller font size of screen titles in Windows 7).

Inservice Training: Added the ability to limit printout of an Employee's training record to the current year, to the past 1 or 2 years, or the entire training history.

Infectious-Disease Surveillance Reports: Expanded the available report formats for Seasonal Influenza and Tuberculosis: Rosters of specified demographic groups (by Employer, by Division, by Department, by Cost Center, or by user-defined Surveillance Group) may be prepared listing all individuals in that group. Each demographic group may be further subdivided (e.g., by Cost Centers within a Department).
Clinical rosters include information about each individual in the group (e.g., whether the individual received inactivated flu vaccine or intranasal vaccine, or whether he/she declined vaccination). Totals for each demographic unit are displayed at the bottom of the roster for that unit. Summary lists show only the totals in each category for each demographic unit.
Compliance rosters contain no clinical information, and indicate only whether the individual is compliant with the surveillance policy. Compliance summaries list only the total number of employees in each unit who are compliant or non-compliant with the policy.
Infectious-Disease Surveillance Reports: Added the ability to prepare compliance rosters which include only the names of non-compliant employees.
Standard Occupational Classifications: Implemented a picklist within each Employee's demographic record which enables documentation of the Employee's SOC, eliminating the need for external lookup and manual data entry.

Inactivity Timeout: Modified the process for changing the number of minutes of inactivity allowed before MediTrax will automatically close (in compliance with HIPAA requirements for electronic medical recordkeeping). This feature is available from the

DOT Examination Form: Modified the third page of the form to include the medical examiner's license number and state of issue. Although this is not required on the official DOT form, clerical staff in some state DOT agencies are now requiring not only a copy of the confidential medical examination form, but requiring that the form includes the examiner's licensing information.

Custom Clinical Protocols: Enabled a user, at the time of scheduling an appointment, to identify specific clinical services which are to be provided (even if a predefined Clinical Protocol has not been selected). This feature was previously available at the time of "checking in" a patient for an Encounter, and several users requested that it be made available when the appointment is first scheduled.
Clinical Protocols: Enabled configuration of clinical protocols for occupational-injury appointments, with the option to bypass the selection of a clinical protocol when scheduling an appointment or registering the patient at the front desk.
